Hospice is a unique and specialized type of holistic care offered for people at the end of their life. Sage Living partners with the team at St. John’s Health Hospice to provide this individualized care to residents. Along with the resident’s physician, Sage Living team, and resident’s family, St. John’s Hospice helps provide comfort and quality of life during a resident’s final journey.

We recognize that this is a very challenging and stressful time for residents and their families but can also be full of reminiscing and deep connection. Our goal is to support the resident and their family and friends on this journey.

Hospice Care at Sage Living includes:

• Meet with a social worker to help plan for end-of-life arrangements
• Bereavement counseling for families dealing with their loved one’s passing
• Music therapy sessions
• Healing Touch sessions – non-contact therapeutic touch as a form of energy therapy
• Medication management to help alleviate pain and anxiety
• Quality of life resident interventions
• Aromatherapy using essential oils
• Ongoing communication with the entire care team to assess and revise care-plan as needed

This special time can look different for everyone. The Sage Living team is here to support residents and families no matter their own wishes, unique cultural, spiritual, and physical needs.
